Nature of Services Root Cause Nurse offers: Functional lab education and interpretation (non-diagnostic) Lifestyle and self-care education Supplement guidance and wellness support via Fullscript Supportive follow-up through virtual platforms This is not a medical service. You are not being diagnosed, treated, or prescribed medication. You are encouraged to consult with your licensed healthcare provider before beginning any new regimen. Scope of RN Wellness Education This nurse operates independently within the RN scope of practice in the state of Florida. All services are intended for health promotion, lifestyle education, and nutritional support based on client-directed lab testing. Acknowledgment I understand that: These services do not replace care from a physician or specialist. No diagnoses or prescriptions will be issued. My participation is voluntary and I can discontinue services at any time. I am encouraged to review all recommendations with my primary care provider.
